A. For new patients, please print and complete the new patient forms from our website. For ALL patient visits, please bring your current insurance or Medicaid cards, plus any prescription medication bottles.

Q. Can I pay my past due balance at my appointment?
A. Yes, we accept cash, personal checks, Visa, MasterCard and Discover cards.

Q. How early should I arrive for my appointment?
A. Please arrive approximately ten (10) minutes early for your appointment so we may verify and update any changes to your personal or insurance information. All new patients should arrive thirty (30) minutes early for appointment to complete paper work and for the staff have time to get everything in the
computer before you see the doctor at your appointed time.
